Sports Physicals

We’re glad you’re here!

At your physical, you will be asked to show a photo ID at check-in for personal protection. We also request, if possible, that all patients complete our Sports Physical Intake Form Sports Physical Intake Form ahead of arrival. Please note that all minor children must be accompanied by a parent or legal guardian. If you are the legal guardian, you must bring documentation proving legal guardianship to place in the child’s chart.
