What is a Bunionectomy and when is it necessary?
A bunionectomy is a surgical procedure used to correct a painful bony bump at the base of a toe joint. Bunions are a progressive foot deformity that can result in pain, swelling, and difficulty wearing shoes. Bunionectomy removes the bump, realigns the joint, and may involve soft tissue repair through open or minimally invasive techniques.
Candidates for bunionectomy
When conservative treatments no longer provide relief, bunionectomy surgery may be the next step. The procedure is typically used for moderate-to-severe bunions causing inflammation, arthritis, or toe crowding.
You may be a good candidate for bunionectomy if:
- X-rays or MRI imaging have shown mild, moderate, or severe bony misalignment
- Pain persists despite non-surgical treatments
- The bunion interferes with walking or daily activities
- Toe alignment continues to worsen over time
- You experience chronic inflammation or difficulty wearing shoes

Tailor’s Bunionectomy (Osteotomy)
Tailor’s bunionectomy targets a bunion on the outer edge of the foot at the base of the small toe. The procedure involves removing or reshaping the bony prominence and correcting the alignment. It is often recommended for patients with mild to moderate bunions with pain along the outer edge of the foot.
Lapidus Bunionectomy (Fusion)
Lapidus bunionectomy targets a bunion at the base of the big toe. The procedure involves fusing the joint to correct misalignment, improve long-term instability, and prevent recurrence. It is commonly recommended for severe bunions and provides strong correction for instability.
Exostectomy
Exostectomy is a minimally invasive procedure that uses a small incision to shave only the bony protrusion, without addressing the misalignment. It is generally only recommended for mild bunions.
It’s normal to have some discomfort after surgery, especially in the first few days. Most patients report manageable pain, with relief from medications, icing, and proper post-op care.
Recovery time varies depending on the procedure performed and the severity of the bunion. Usually, initial healing takes a few weeks, though swelling may last for several months. Full recovery and return to normal footwear can take a few months.
Following your surgeon’s instructions is key to protecting the correction and avoiding complications.
In many cases, patients can begin walking shortly after surgery, often the same day or within a few days, using a protective boot or shoe. However, weight-bearing guidelines vary by procedure, and some surgeries require limited pressure on the foot initially.
Yes. Surgery is typically considered only after conservative treatments have been explored. Non-surgical options may include:
- Wearing wider, supportive footwear
- Using orthotics or padding to reduce pressure
- Activity modifications
- Anti-inflammatory medications
- Icing to reduce swelling
While these approaches can help manage symptoms, they do not correct the underlying deformity.
