Patient Portal

Everything In One Spot

Existing and new patients, we encourage you to use our client portal to fill out paperwork and make changes to your information. Our EMR (Electronic Medical Record) system allows you to access your medical information. It also helps your healthcare providers make better clinical decisions and share important information with others involved in your care – including your hospital. The system uses the most advanced encryption technology to keep your records safe and secure.

How To Use The Portal

First Time Users – Click here and follow the instructions. This can only be done within 7 days prior to your first appointment (or your next appointment if you’re already a patient).

Access Your Records – Once you’re registered, you can enter the Patient Portal at any time to read and print your office notes.

Change Medical History or Demographic Information – Log in to the Patient Portal and follow the instructions. This can only be done online within 7 days prior to a scheduled appointment. If you want to make changes to your information at other times, please call our office at 603.431.1121.


24-Hour Refill Line

Our Prescription Refill Line is available for patient use 24 hours a day at 603-559-8329. Messages are picked up during appointment hours only and there is a 24-48 hour turn-around time (48 hour turn-around time for weekends and holidays). We will address your prescription refill as soon as possible. Narcotic prescriptions must be picked up in person at our office by 4:30pm. A valid form of picture ID is required for all narcotics prescription pick-ups.
